For moderate levels of DD on the T1-DDAS or T2-DDAS, the provider will discuss the main sources of distress with the patient and determine if an e Consult or referral might be indicated based on the needs of the patient in addition to the AI chatbot. An e Consult to the following specialists can include endocrinology, clinical pharmacy, social work or care management, diabetes education, and behavioral health.

Improving Patient Access to Hospital Pharmacists Using eConsults: Retrospective Descriptive Study
In a 2015 systematic review by Vimalananda et al [10], 27 studies on e Consult services were reviewed. The authors found high levels of provider satisfaction (70%-95%), faster replies (less than 3 days), and a decline in specialist referrals. Barriers to implementing an e Consult tool include low levels of patient engagement and awareness, staff inexperience and limited availability, and lack of suitability for all patient groups and financial obstacles [7].

However, electronic consultation (e Consult) can greatly improve access to specialist advice in many cases by allowing requesting providers, usually PCPs, to communicate directly with specialists electronically regarding a patient’s care, often avoiding the need for a traditional consultation. Studies of e Consult services have found that they improve access, lower costs, and deliver high rates of patient and provider satisfaction [7,8].

Primary Care Psychiatry eConsults at a Rural Academic Medical Center: Descriptive Analysis
The responding psychiatrist reviews the specific consultation question and information in the e Consult referral template and may also choose to review additional clinical information available in the EHR, such as the PCP’s most recent progress note. An example e Consult is shown in Textbox 1, with a standard disclaimer included at the end of every e Consult in Textbox 2. Response time is expected within 3 business days (72 hours) and both the request and response become part of the patient’s EHR.
