Persons with a chronic health condition, such as the bleeding disorder hemophilia, are encouraged to use e Health tools to enable personalized treatment and shared decision-making. Hemophilia is an X-linked inherited coagulation factor deficiency of factor VIII (in hemophilia A) or factor IX (in hemophilia B), resulting in an increased bleeding tendency.

To improve health outcomes, stakeholders have advocated for the incorporation of shared decision-making (SDM) that encourages physicians to involve patients in selecting therapeutic treatments, among others [11]. SDM improves patient behavior and results in higher adherence to physician guidance, leading to improved health outcomes [12,13]. Currently, there are SDM tools available to facilitate decisions regarding nonvalvular atrial fibrillation anticoagulant treatment [14-18].

Thompson [14] has described patient participation as covering five levels of patient-determined participation or involvement: noninvolvement, given information, dialogue, shared decision-making, and autonomous decision-making. Thompson [14] conceived of professionally determined patient involvement as being along a power continuum from a low level of patient power (exclusion) to a high level of patient power (informed decision-making) in patient consultations.

Shared decision-making (SDM) has been promoted to support patients in making informed decisions that best fit their personal preferences, circumstances, and concerns [1,2]. This is particularly important in oncology as many treatments involve serious side effects, and treatment decisions involve a trade-off between benefits and risks [3,4]. Approximately 110,000 Dutch patients are diagnosed with cancer each year [5].

We included medication adherence because this is related to the knowledge about the condition and treatment and shared decision making and may be facilitated by having an overview of the medication list [42,43].
The secondary outcomes are patient-reported changes in use of GP services (number and type of contacts), patients’ confidence in their communication with the GP, knowledge about the disease, satisfaction with GP care, and patient-reported perception of changes in the doctor-patient relationship.

According to the Ottawa Decision Support Framework, people cannot make preference-based shared decisions without accurate knowledge of the options, an understanding of what is most important to them, and effective support from others [15]. In the decision to stay at home or relocate, different kinds of knowledge (eg, medical, social, financial, and familial) are needed and supplied by different stakeholders.

During the shared-screen review process, the clinicians can answer patient's questions about each medication and provide an opportunity to adjust certain medications to address patient concerns such as cost and side effects.
